Fax cover sheets should contain all of the pertinent identifying information for a document to be transmitted by facsimile (fax), and are always the very first page of any fax transmission.
Although faxes are not sent at the same rate they were a decade ago, they are still required in some situations. For example, being able to fax certain documents directly to my bank sometimes saves me from having to make a special trip to the bank. Some companies that I deal with (eg. insurance companies) still require faxed documents to verify certain facts about me and my business.
When sending a multi-page fax it is recommended that, on the cover sheet, you indicate the total number of pages in the transmission, including the cover page. This allows the recipient of your fax to make sure that they have received all of the pages at their end.
