Shipping Label Sample



Shipping labels provide important descriptive details about the contents and routing of the package(s) to which they are affixed; normally, one shipping label is attached to each individual package being shipped to a customer.

The key elements of a shipping label are: destination address, originator return address, purchase order number, shipping date, and contents of package. Purchase order number in particular is very important because it provides a key cross reference for the intended recipient of the shipment.

Clear and descriptive shipping labels are very important for customs inspection when you are shipping packages internationally. Also, if your package is going to a large organization, the better the shipping labels, the better the chance of your package reaching the intended recipient.

This type of return mail order form is often inserted into business mailings going out to regular customers in order to entice them into ordering additional products or services. They often involve some sort of special offer as an incentive to fill out the form and mail it in.

Order Form – Book Insert Sample


Order forms are often inserted into books or magazines to entice readers to fill out the form and mail it in to receive a product or special offer.


Order forms are often found as insert pages near the back of paperback books. Their purpose is normally to try to get the reader to order another book by the same author, or books of similar theme by other authors. In some cases they can also act as an advertisement for a new book or product.

Book insert order forms are usually of the blank form variety, allowing the user to fill in the various order and customer-related fields. The page must be made easy to tear out for easy mailing. It is important to also provide alternative order channels on the form such as toll free numbers and e-mail order addresses.
